On Ozempic and Can’t Poop? 🚽

You're taking Ozempic or Wegovy. You're eating less, feeling full sooner, and maybe seeing the weight loss you hoped for.

But there's a problem.

You're barely pooping, your belly feels bloated, and adding more fiber doesn't seem to help.

Sound familiar?

I see constipation often in patients taking GLP-1 medications. And there's something I wish more people understood:

Constipation isn't always about eating too little fiber. Sometimes, the bigger problem is how slowly your gut is moving.

Let's talk about it.

Why GLP-1 Medications Can Make You Constipated

GLP-1 medications help you feel fuller by slowing stomach emptying. They can also affect how quickly stool moves through your intestines.

When stool stays in your colon longer, your body continues absorbing water from it. That leaves harder, drier stools that are difficult to pass.

In clinical trials, 24% of people taking Wegovy 2.4 mg reported constipation, compared with 11% taking placebo. These rates are specific to that medication and dose.

And there's another issue: you're eating less.

Smaller meals can mean:

  • Less food available to form stool.
  • Less fiber to support regular bowel movements.
  • Less water, making stools harder.

But why doesn't more fiber always help?

Think about adding more cars to a traffic jam.

That's what can happen when you suddenly increase fiber while your gut is already moving slowly.

You may end up with more bloating, gas, and discomfort.

Fiber matters, but we need to understand the traffic jam before adding more cars.

I usually start by looking at water intake, how much someone is eating, stool consistency, and bowel movement frequency.

From there, we can decide what changes make sense. Sometimes that includes medication for constipation, with guidance from your healthcare team.

Your Gut Takeaway

If you’re dealing with constipation while taking GLP-1 medications, start with three habits:

  1. Hydrate. Fiber needs enough water to work properly.
  2. Add fiber gradually. Aim for an amount and type that fits your symptoms and needs.
  3. Get moving. Even a short daily walk can help support bowel regularity.

Constipation often improves as your body adjusts, but don’t ignore ongoing symptoms. Severe abdominal pain, swelling, vomiting, or inability to pass gas requires urgent medical care.

Remember: GLP-1 medications can be a great option for many people. You shouldn’t have to feel miserable while working toward better health.
